Fill the necessary details such as Membership Number, aadhaar Number and your Aadhaar linked
mobile number.
आवश्यक वववरण जैसेसदस्यता संख्या, आधार नंबर और अपना आधार सेजुडा मोबाईल नंबर भरें|
Click Get OTP and enter the OTP, you receive on your registered mobile number, and then select
verify OTP.
गेर् ओर्ीपी पर क्ललक करेंऔर अपनेपंजीकृ त मोबाईल नंबर पर प्राप्त ओर्ीपी दजटकरें, और वफर सत्यावपत ओर्ीपी
चुनें|
Upon login into the depositor registration, the depositor details entry screen will appear.

The first step shows UIDAI Declaration Screen. Go throught the declaration and Accept the
Terms and Conditions option

Move to next Page after Accepting the terms and conditions
वनयम और शतेस्स्वकार करनेकेबाद, अगलेपृष्ठ पर जाऐं |
The Personal Details Screen will appear next.
आगेव्यक्लतगत वववरण स्रीन ददखाई देगी |
1. Enter 12 Digit Aadhar Number and click on Get OTP. Depositor will receive OTP in the
registered Mobile Number
१२ अंको का आधार नंबर दजटकरे और गेर् ओर्ीपी पर क्ललक करें| जमाकताटको पंजीकृ त मोबाईल नंबर पर
ओर्ीपी प्राप्त होगा |
2. Enter OTP and Verify. The display shows Aadhar user details like First Name , Middle Name,
Last Name, Date of Birth and Father/ Husband Name.
ओर्ीपी दजटकरें, और वफर सत्यावपत करें| वडस्प्लेपर आधार उपयोगकताटवववरण जैसेप्रथम नाम, मध्य नाम, अंवतम
नाम, जन्म वतथथ और वपता/पती का नाम ददखाई देगा |
3. It is optional for the User to enter their Email id.
उपयोगकताटकेथलयेअपना इमेल आईडी दजटकरना वैकक्ल्पक है|
4. Enter Alternate Mobile Number and press on Save Mobile and Email
वैकक्ल्पक मोबाईल नंबर दजटकरेंऔर सेव्ह मोबाईल और इमेल पर क्ललक करे
5. Press Next Button
अगला बर्न दबाऐं
6. Now Enter Claim Details and upload document
अब ललेम वडर्ेल्स दजटकरेऔर दस्तावेज अपलोड करे|
Click on Add Claim Button and you will see all details in the below grid and press next once
all Claim details are added. In this screen multiple claim details can be added.
ऍड ललेम बर्न पर क्ललक करेंतब आपको नीचेददए गए विड मेंसभी वविंवरण ददखाई देंगेऔर सभी ललेम
वडर्ेल्स समाववष्ट करनेके बाद अगलेबर्न पर क्ललक करें| इसमेस्रीन मल्र्ीपल वववरण समाववष्ट वकयेजा
सकतेहै|
7. Download Prefilled Claim Request Form
पहलेसेभरा हुआ ललेम अनुरोध प्रपत्र डाऊनलोड करें|
8. Upload the required Documents
आवश्यक दस्तावेज अपलोड करे|
Upload Prefilled Form ( Less than 3 MB file is only allowed) with Sign and click Finish
हस्ताक्षरकेसाथ पहलेसेभरा हुआ प्रपत्र( के वल ३ एमबी सेकम फाईल की अनुमती है|) अपलोड करेऔर
वफवनश पर क्ललक करे|
Thank you Page will come with a Claim Request Number
